What is Hydrocele (Fluid Accumulation in Scrotum)?

Hydrocele increases of fluid present in a small amount around the testis.


What are the Causes of Hydrocele?

There are two types of hydrocele: Primary and Secondary.

Primary hydrocele occurs due to continuous relationship between the scrotum and leading the abdominal flow into the scrotum after birth. Mainly, inguinal hernia is observed in these cases.

Secondary hydrocele occurs due to imbalance between secretion and absorption of the fluid for various reasons. Scrotal infections, injuries, trauma, testicular torsion, varicocele surgery and testicular tumors can result in hydrocele.



What is the Frequency of Hydrocele?

It is observed around 84-90% in newborn period, and 20% in adults. It is more frequent in preterm births and the elderly people than 50 years old.


How is Hydrocele Diagnosed?

The diagnosis can be made easily by physical examination. There is a smooth, firm, straight scrotum structure. It is an important finding that observing the shadow from behind the scrotum by shining light in a dark room.


What should be Considered in the Diagnosis of Hydrocele?

Since hydrocele forms a mass in the scrotum, testicular mass should be considered in differential diagnosis. (In addition, ultrasonography can be extremely helpful in the diagnosis).


How is Hydrocele Treated?

There are different treatment methods for hydrocele:

  • Needle aspiration: However, there is a possibility of relapse with relapse with this method.
  • Needle aspiration+Sclerosing agent injection: Relapse is observed less frequently with this method. However, this method is not recommended for neonatal hydrocele. In cases which this method will be applied, there should not be additional problem such as hernia.
  • Surgical procedures: They are the most successful treatment methods where there is no possibility of recurrence.


When should Hydrocele be Treated?

Hydrocele detected in the newborn period can be waited until the end of first year of life if there is no hernia. Hydrocele observed in elderly can also disappear spontaneously between 6-9 months. Treatment is postponed within this period.


What are the Complications of Hydrocele Treatment?

Complications of hydrocele surgery are extremely rare. Testicular damage has been reported as 0.3%

What is the Effect of Hydrocele on Reproduction?

Excessive amount of fluid around the testicle can increase the testicular temperature and can affect sperm production.  

What is the Effect of Hydrocele on Sexual Function?

Hydrocele does not affect the hormone secretion (testosterone production) function of the testicle. However, large hydrocele can result with difficult sexual intercourse.
